- A referral is not required to set your first appointment. Once the first appointment is complete, a plan of care will be sent to a doctor of your choice for approval.
- How long does it take to make an appointment?
- Clients are able to get appointments within the same week.
- How many visits will I need?
- Visit recommendations will depend on your exam findings. Your therapist will discuss care options and create a plan with you.
- Do you take insurance?
- Awaken Pelvic PT is an out-of-network provider. Each session is paid for at the time of service and a Superbill will be provided. The Superbill may submitted to your insurance company for coverage under out-of-network benefits by insurance with the exception of Medicare, VA, and Tricare. Awaken Pelvic PT does not guarantee reimbursement by insurance companies.
- What forms of payment do you accept?
- Services can be paid for by cash, debit, credit, FSA (flexible spending account), HSA (health savings account), and CareCredit.
